The Client Services department provides care coordination and support services to individuals living with various chronic conditions. All services are delivered using a client-centered care model and prioritize involvement of the client and their existing natural supports in service planning.
Health Homes care management services are available to Medicaid beneficiaries with qualifying chronic illnesses. Adults who have HIV/AIDS, diabetes, hypertension, mental health diagnoses, asthma, Hepatitis C, and a variety of other chronic conditions may be eligible for these services.

This pilot utilizes grant funds, realized by Medicaid Redesign Savings, to provide supportive housing services. Medicaid beneficiaries who are unstably housed and have utilized emergency or inpatient medical services multiple times in the last year may be eligible. This program provides for short-term rental assistance and case management services.
